Three years ago, NYPD detective Max Payne's wife and baby were murdered. Max gets himself transferred to the cold case office where he can continue searching for the killer who got away. He's a loner, but two people reach out to him during a fateful week: Alex, his ex-partner who may have found a clue, and BB, the security chief at the pharmaceutical company where Max's wife worked. Meanwhile, bodies are piling up, some as a result of a drug on the street that is highly addictive and, for many who take it, brings hideous hallucinations. When one of the bodies is a woman Payne was the last to see alive, her sister comes looking for him armed to the teeth; Max must move fast.

nudity Max enters a night club of sorts where he sees several people (mostly female) scantily clad and acting in a rather seductive fashion. He meets what we assume is a girl who makes her money from prostitution and she attempts to seduce him, we see her topless and in her briefs from behind, and later only covered by a small bed sheet, and as she gets up from the bed the of her right breast can be seen.
violence The theatrical release is not that violent, with blood being minimal, although the film does have a dark tone. The unrated version however is very bloody, with some graphic and large amounts of blood spurting from enemies when shot. A man blows stall doors open with a .410 gauge revolver while a man tries to crawl away While experiencing hallucinations a man is hit by a train (very brief) Several mild fistfights throughout A man with a sword stalks a woman and she is attacked by him (she sees him as angel-like creatures) ^a crime scene later on explains she was torn apart (this is mostly in dialogue, mutilation is not shown A man enters his apartment to find a body (his throat is slit with no blood but wound on neck is visible) A man stabs a drugged person (shadows are seen, along with slicing/crunching noises) A man falls off a rooftop and lands on top of a car (it is intense, but no blood is shown) An office room is torn apart by gunfire (few people are shot, and only one is clearly killed) Henchman are practically blown across rooms by shotguns, and machine gun fire riddles many people with bullets (no blood in either occasions, but one or two people have visible gunshot wounds with some blood) A man is shot in slow motion, and a puddle of blood forms around his body.
profanity One F-word derivative (in the unrated version of the film, the F-word is used several times) and ten uses of "shi*". "Jesus" is used a half-dozen times. God's is combined with "damn" twice. There are three or four uses each of "ass," "hell" and "bitch." One crude term for a penis is used. The Unrated version contains more profanity than the Theatrical. Around 7-8 uses of F*ck and 3 uses of Motherf*cker. Some are hard to hear.
alcohol The storyline is based on a fictional drug called Valkyr, that has two opposite side effects, however both make the drug addictive: 1. Causes mildly frightening hallucinations that mostly result in fairly gruesome deaths (this is shown several times) 2. Increases aggression and makes gives the host a need to kill (The main villian is subject to these side effects and his acts are shown many times)
frightening Much of the action is intense whilst scenes Of Max finding his dead family may spook the very young. These instances aren't graphic but it is heavily implied that a women and child have been murdered and the effect it's had on Max. The sequences where people are under the influence of Valkyr are also fairly intense and feature some frightening imagery including winged demons and fire.
